Public HealthReports: Fentanyl Drives Sharp Increases in Pediatric Opioid Exposures

A recent study published in Public Health Reports, “Trends in Pediatric Opioid Ingestion and Associated Factors in Washington State, 2014–2023”, examines the growing impact of fentanyl on pediatric opioid exposures. Researchers found that fentanyl has become an increasingly significant driver of serious opioid ingestions among children and adolescents, underscoring the need for continued prevention and public health intervention.

Researchers from the University of Washington School of Public Health and the Washington Poison Center analyzed 2,873 nontherapeutic opioid ingestions among children and adolescents ages 0–17 between 2014 and 2023. The study found a 9,700% increase in reported fentanyl ingestions over the 10-year period, while hydrocodone and oxycodone ingestions declined substantially. Fentanyl exposures were also associated with significantly more severe outcomes, with children experiencing more than four times the odds of life-threatening effects compared with exposures to other opioids. Nine deaths were reported during the study period.

Patterns of exposure varied by age. Nearly all ingestions among young children (0 – 3 years) were unintentional, suggesting household access and unsafe storage as key risk factors. Most adolescent exposures were intentional and frequently associated with suicide attempts, underscoring the intersection between the opioid crisis and youth mental health challenges.

The findings mirror broader national trends in the opioid epidemic, where illicitly manufactured fentanyl has become the dominant driver of overdose morbidity and mortality. The authors state that prevention efforts should include safe medication storage and disposal, expanded naloxone availability, enhanced surveillance, and interventions tailored to the distinct risks facing young children and adolescents. They also emphasize the need to address the disproportionate burden of severe opioid exposures in socioeconomically disadvantaged communities.

The study offers important state-level evidence to inform public health policy, injury prevention efforts, and targeted interventions to reduce opioid-related harm among children and adolescents.
