Public Health Story Collection Form

We are collecting powerful, real-world stories that show how public health impacts daily lives. These stories will be used in a national report, congressional advocacy, media outreach, and social media campaigns, and we invite our members and others to use this form to tell their own personal stories or those of the community members they work with on a regular basis.

We are seeking stories from those who benefit from public health—families and community members, as well as public health professionals working on the frontlines and our members. Your stories will help us change the narrative about the importance of public health funding and programs at the local, state, and national levels. Now more than ever, it is critical to amplify the importance of public health.
